10-01-2009 Effective date; 01-27-2011 Amended "indicate form of delivery medium" to "indicate form of broadcast medium" to make clear that the entry is not intended to include hard goods, e.g., DVDs, CDs, etc. On 5-2-2013, this 1-27-2011 entry was amended to delete the words, "or live" from the end of the ID because it created an ambiguity as to its meaning.
